The dot is in the incorrect position

This is due to two reasons - the first is that a teacher has updated a results after you ran Build in the Quick Setup window. Go to the Quick Setup and run build and this will fix the problem. The other is caused by the teacher giving the lowest score possible for a dimensions. This will give the student a dot in the "D" position even though the score for any alternative dimension would be a higher result. In this case, the graphic is correct. This rule is based on the government VELS specifications and will gradually stop as more domains as assessed via the Australian (AusVELS) Curriculum.
